**YOU MUST HAVE YOUR ATHLETE’S SCHOOL ID NUMBER AVAILABLE AS IT IS REQUIRED ON EACH FORM**

• Physical Exam Instructions: This form must be printed and taken to the physician’s office for him/her to fill out. Remember that you CANNOT submit the physical form online. It must be filled out by a physician and returned to your school Athletic Trainers. Physical forms are available from the athletic trainers, or can be located at https://www.uiltexas.org/files/athletics/PrePhysFormRvsd2024.pdf All other forms listed on this website should be submitted electronically.

• To access the online forms hold your cursor over the “Electronic Participation Forms” tab. You will get a drop down list of the forms.

• To begin, click on the form name and fill out the information requested. If you do not know the answer to a question, please answer the question with N/A or none.

• To sign the document, click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over click on the refresh icon next to the signature box. If you are using a touch screen device, you may create an electronic signature, by holding your finger to the screen and signing your name or by using a stylus.

Remember: It is important that you and your child fill out and sign all required spaces. Forms will not be accepted electronically if the forms are not completed correctly.

• You will receive a confirmation email once the document has been reviewed by the Athletic Training Staff.
